When it comes to Advance Care Planning (ACP), getting patient preferences into the EHR is just the starting point. Been thinking a lot about my takeaways from Florida Association of ACOs (FLAACOs) so here is a slightly delayed recap — and an unsolicited all-time great movie reference: Jonathan Bush from Zus Health kicked off his keynote with a Matrix analogy: taking the red pill versus the blue pill. In his version, the “red pill” represents the foundational work ACOs have been doing to survive—risk adjustment, coding, the blocking and tackling of value-based care. The “blue pill,” on the other hand, is where the future lies: deeply understanding your data, managing populations proactively, and using those insights to drive outcomes. That theme, turning data into action, echoed throughout the conference. In my session, we built on that idea by showing how understanding your data reveals exactly where care quality or cost gaps exist, and how digital tools can bridge them. One message stood out: value-based care is accelerating. Leaders are moving beyond pilots and foundational infrastructure. They’re building systems that predict, personalize, and proactively intervene. It was energizing to connect with so many founders, operators, clinicians, and policy thinkers pushing the boundaries of how we deliver better, more equitable, and more sustainable care. The momentum is real — and the next phase will favor those who can blend operational rigor with digital-first population intelligence. Already looking forward to next year — and to watching the industry choose the “blue pill” at scale.

My parents are in their late 70s. They just had their annual primary care visits. The next available appointment? 2027. They live in rural New Hampshire, a town of 300 people, and already drive over 30 minutes just to see their primary care doctor. The access issue is real. This is rural healthcare in America. We talk a lot about value-based care and better outcomes but none of it works if people can't actually see a doctor. Provider access is the foundation everything else is built on. I'm encouraged by initiatives like the Rural Transformation Fund and companies tackling this head-on. But the loss of ACA subsidies and potential Medicaid cuts are going to be even more devastating to these communities. My parents are fortunate enough to be able to leave the community for care if needed. Many of their neighbors and families across the rural areas of our country don't have that luxury.

Each day we encounter families facing moments that matter. For one family, that moment came when a patient’s spouse called us in distress. Her husband, living with end-stage renal disease, had chosen to stop dialysis. Hospice care had been ordered, but no one had called back and time was slipping away. Our patient advocate team stepped in right away, coordinating directly with the hospice provider. By that afternoon, care was in place at home — just as the patient had wanted. “It was such a relief to know we weren’t alone. Koda made sure everything happened quickly so my husband could receive the care he wanted, right here at home.” Behind every referral and every document is a family — trying to do right by someone they love. That’s why we do what we do.

At Koda Health, we often meet people in their most vulnerable moments — when love, fear, and responsibility collide. One patient’s daughter recently called us about her father’s advance care plan. He was in the early stages of dementia and had documented his wishes, but she was struggling to accept what those choices meant. “I know what my dad doesn’t want, but I don’t know how I feel about some of those choices for him.” Through Koda’s guided tools and educational videos, she found the clarity to talk openly with her father — and the confidence to honor his voice. In the process, she decided to complete her own plan too. It’s a reminder that advance care planning isn’t just about documents. It’s about conversations — honest, emotional, and human — that bring families closer together.
