WOW! Creativity and Culture to the NINEs! The InnovAge San Bernardino center OUTDID themselves with their huge Halloween celebration. Anyone taking notes? And our Pueblo center put on a wildly colorful and deeply meaningful event. Dia de los Muertos or “The Day of the Dead” is a Mexican holiday celebrated November 1-2 where families honor deceased loved ones with altars, marigolds, and favorite foods in a festive atmosphere. The celebration includes both remembering the dead and sharing gifts like sugar skulls and Pan de Muerto with living friends and family. And to our Richmond team - Snow White and her dwarves stole the show! Thank you to ALL of our teams for their incredible creativity and commitment to making InnovAge a fun place to be! #CrushedIt #Halloween #DiaelosMuertos #Culture #Creativity #InnovAge PACE

What an incredible day celebrating with our Tampa Bay community! Our Grand Re-Opening of InnovAge Florida PACE–Tampa was more than an event—it was a powerful reminder of what's possible when we come together for our seniors. Neighbors, partners, and friends joined us to honor our veterans on Veterans Day and recognize the caregivers who give so much during National Family Caregivers Month. Howe, one of our participants and a veteran, stole the show, sharing his personal story of how InnovAge PACE and Tampa General Hospital have transformed his life and care. His words reminded us all why this work matters so much. A heartfelt thank you to Tampa General Hospital for being such an amazing partner in this mission. Together, we're making it possible for more seniors across Hillsborough, Pasco, and Hernando counties to live independently at home with the comprehensive care and support they deserve. We're so grateful to Florida Governor Jeb Bush, State Representatives Diane Hart and Karen Gonzalez-Pittman, Luis Laracuente from U.S. Senator Rick Scott's office, and Matt Hudson from the Florida PACE Providers Association for joining us and championing this vital work for our seniors. Thank you to the incredible Cienna Wesley, BS, MPH for sharing your beautiful voice and uplifting spirit with us—your patriotic performances touched every heart in the room. And to Feeding Tampa Bay, The Salvation Army Tampa, and Seniors in Service of Tampa Bay, Inc.—your work inspires us every day. We're honored to support you. To everyone who joined us, performed, spoke, and celebrated—thank you for being part of our village. This is what community looks like. #SeniorCare #TampaBay #VeteransDay #CaregiverAppreciation #PACE #CommunityFirst

“This is the model. This is what it should look like,” said Colorado State Representative Jacque Phillips while touring the InnvoAge Colorado PACE center in Thornton. As budgets tighten and senior support programs face growing pressure, more lawmakers are seeing PACE (Program of All-inclusive Care for the Elderly as a solution that continues to stand out. “Our goal is to keep participants at home, safe, and independent,” said Linda Whyte, Area Center Director for InnovAge PACE in Thornton. On FOX31’s Great Day Colorado, she spoke about the increasing number of lawmakers touring her center and coming away impressed and inspired. “By providing preventive care, after-hours nursing, transportation, and social support — all under one roof — we’re reducing ER visits, hospitalizations, and caregiver stress.” “You have to come and see (InnovAge PACE) for yourself,” said Colorado State Representative Michael Carter, on a tour of InnovAge’s Aurora center. “When we talk about budgets and numbers, what we really need to focus on are services — the government’s ability to care for its citizens. We need to talk about the services that you guys are providing.” “I want this to be available to everybody. Just in my neighborhood, we have a lot of health care concerns,” said Phillips. “Really going through this place today, I thought this is the model. This is what it should look like.” Watch the full Great Day Colorado segment here: https://lnkd.in/g4j-5u3r #InnovAgePACE #ColoradoSeniors #SeniorCare #TalktoLocalLawmakers

A spotlight on our InnovAge Florida PACE Tampa team who showed heart and spirit at this year's Alzheimer's Walk in their area! Led by Team Captain Terry Lindstam, our amazing walkers—Therese Melendez, Michelle Keyes, Nancy Patterson, and Charity Miller—raised an impressive $1,528 for Alzheimer's research and support. A special shoutout to Jeremy Welleaur, who single-handedly managed our "spirit table"! To all our our teams who came out to show your support - thank you for your dedication, compassion, and commitment to making a difference in the lives of those affected by Alzheimer's - many of whom are the very seniors we care for every day. #EndAlzheimers #WalkToEndAlzheimers #InnovAge #FloridaPACE #CommunityImpact #AlzheimersAwareness #TeamInnovAge #MakingADifference

This is not just a picture. It's the start of an amazing story. Recently, at our InnovAge California PACE center in San Bernardino, one of the seniors in our program (a participant) went into cardiac arrest on the bus. Our CNAs immediately started CPR on that bus. Nurse Practitioners Christine Penaflorida and Clint Hart—the two you see here—dropped everything and rushed to the bus. They ran the code right there with our nursing team. EMS arrived, continued compressions, gave epinephrine, and got our participant to Loma Linda Hospital. Our participant made it. "Today is a great day to save a life... and we did," said Center Medical Director Dr. Bob Chiang. This is what happens at InnovAge PACE centers—not just sometimes—every day. CNAs, nurses, doctors, drivers, all working together without thinking twice when someone needs them. It's the kind of teamwork you can't fake. It's just who our people are. We're so proud of Christine, Clint, and the entire San Bernardino team. This happens because of people like them at every single one of our centers. People who show up, know their stuff, and care deeply about the seniors we serve. That's InnovAge. #InnovAge #PACEProgram #HealthcareHeroes #CaliforniaPACE #SeniorCare #InlandEmpire

At our Henry Avenue center in Pennsylvania, participants and staff showed up and showed out for those who have fought, are still fighting, and have lost their battle to breast cancer. They held a special event honoring our survivors, and together we colored the center pink with love and hope. Our Colorado PACE Aurora center and Pennsylvania LIFE center also joined in, creating beautiful moments of awareness, education, and support. And our Orlando team also participated in a breast cancer 5K and proudly representing InnovAge in the fight against this disease. Thank you to all of our incredible InnovAge teams and participants for making this month so meaningful ❤️ #BreastCancerAwareness #InnovAge #ThinkPink #BreastCancerAwarenessMonth #CancerSurvivors #PACEProgram #TogetherWeStand #CommunityStrong

Important work by our CEO Patrick Blair - joining healthcare leaders at the 2025 Innovations in Value-Based Care Conference in NYC. The panel explored something close to our hearts: bringing care to patients where they live—at home and in their communities. This is exactly what we do every day through our PACE programs, helping seniors live independently while getting the care and support they deserve. #ValueBasedCare #HealthcareInnovation #PACE #InnovAge
I had a great time joining my fellow healthcare leaders at the 2025 Innovations in Value-Based Care Conference in NYC last week to discuss one of the most exciting shifts in care delivery: creating value beyond the clinic. Our session, “Expanding Reach: Creating Value Outside of Clinical Settings,” explored how value-based care models are helping patients. Through innovative approaches, we are creating models that lead to better outcomes in the places where patients spend most of their lives: at home and in their communities, as they want. At InnovAge, we see this every day through our PACE programs, which support seniors in living safely and independently while receiving the care and coordination they need. It’s inspiring to see how technology and community-based models are converging to make that possible. Thank you @Sumir Sahgal, @Benjamin Kaska, @Sharon Samjitsingh, and @Juliette Price for an informative and important conversation! #valuebasedcare #Innovation #healthcare #HealthcareInnovation #PACE #IntegratedCare #PopulationHealth #HealthEquity #InnovAge #IVBC #HSG

"A lot of our participants don't really feel like cooking anymore," says Melissa Mayer, a dietitian with InnovAge Colorado PACE Aurora.. "Maybe they're living alone, or it's just hard to get motivated. So when they come in for lunch, they're not just getting a balanced meal—they're getting to sit with friends, laugh, catch up. That part matters just as much." And honestly, she's right. "Eating alone at home every day is kind of isolating," says InnovAge Pueblo Center Medical Director Dr. Rona Knudsen. "But sharing a meal with people? That changes things." Hear more in this FOX21 Loving Living Local segment: https://lnkd.in/gAHAp8vF At InnovAge, it's not just about the food (though the dietitians do make sure it's tailored to what each person needs). There's a whole team—doctors, nurses, therapists, social workers—who work together to help people stay healthy and keep living at home as long as possible. "I know the food is good for me, and if something doesn't work, my dietitian tweaks it," says Jodie, an InnovAge Aurora center participant. "I have more energy now. I actually look forward to going." Hear why Jodie says she can't talk about InnovAge without tearing up in this informative and touching FOX31 Great Day Colorado segment: https://lnkd.in/gAHAp8vF If you or someone you know could use this kind of support, it might be worth checking out.
